I authorize and reference or churches listed in this application to give you any information they may have regarding my character and fitness for volunteer ministry. VOLUNTEER MINISTRY STANDARDS:

QUALIFICATIONS FOR ALL REVIVAL TEMPLE VOLUNTEERS WORKES: Christians who are in places of responsibility in the church are required to be examples in faith, conduct, and business affair. One of the best ways to present Christ to the people of our community is by maintaining a high standard for workers. The following guidelines will be required of any person who works in any of the ministries at Revival Temple

1. Be in agreement with the ‘Statement of Faith’ of Revival Temple.
2. Have been a faithful member of Revival Temple for a minimum of six (6) months.
3. Make a minimum of six (6) month commitment.
4. Complete this application for volunteer ministry.
5. Be loyal to the Pastor and leaders of Revival Temple.
6. Be faithful to your assigned positions.
7. Live a separated Christian life.
8. Attend all worker’s meetings.
9. Be faithful to attend all regular church services.
10. Give at least three (3) days’ notice if you know you will be absent.
11. Be at your designated post thirty (30) minutes before starting time.
12. Be neat in your appearance.
13. Complete appropriate workers’ training course(s) required in your area of ministry.
14. Your home life must be in order.
15. Give thirty (30) days’ notice when resigning your position
16. If you have ANY unusual occurrences, notify the Pastor or Elders of the Church immediately.
17.I f you have any problems or complaints, notify the Pastor BEFORE OTHERS!

AGREEMENT:

I have read the above qualifications and I am in full agreement with them. I pledge to keep them to the very best of my ability. I clearly understand that failure to keep any of the above requirements is grounds for dismissal.
